Issues with trying to activate my phone

I'm trying to help my uncle swap his phone after his old phone broke down. I get through most of the process but after picking out a plan and going to activate, the plan details on the new phone are missing and the activate button won't work when I click it. If anyone knows anything about this or how to help, I'd really appreciate the help.

Let’s get this thing worked out. Can you please send me a private message with your phone number, PIN number, DEC/IMEI of the device and the SIM card number to activate?
