No Service, No Signal

About three or four days ago, I noticed that while sitting at work (a place I have had perfect service up until now - for the past five years) I had no signal on my phone.  I contacted VM support and was given a code to plug into my phone to 'reconnect' to the network.  This worked for a day or so, but the problem has returned and persists even using the code the rep had me write down to 'reconnect'.  I don't know if it's a tower issue or what, but suddenly coverage in my area where I have had NO PROBLEMS over the past several years is non-existant.  Would love to know what is going on and who I can contact to report this so I can continue to use my phone.

Additional Information:

On my phone, by the signal bars, it keeps jumping between LTE and 3G back and forth over and over and over at 0 signal.
